Informed Consent, Release Agreement, and Authorization 

I understand that participation in festival activities (including camping) involves the risk of personal injury, including  death, due to the physical, mental, and emotional challenges that may occur. Information about festival activities may be obtained from the information booth, activity coordinators, or event staff. I also understand that participation in these activities is entirely voluntary and requires participants to follow instructions and abide by all applicable rules and the standards of conduct set forth in the Northern Colorado Covenant of the River code of conduct. 

In case of an emergency involving me or my child, I understand that efforts will be made to  contact the individual listed as the emergency contact person by the medical provider and/or festival staff. In the event that this person cannot be reached, permission is hereby given to the  medical provider selected by festival staff to secure proper treatment, including but not limited to hospitalization, anesthesia, surgery, or injections of medication for me or my child. Medical  providers are authorized to disclose protected health information to festival  medical staff, festival management, and/or any physician or health-care provider involved in  providing medical care to the participant. Protected Health Information/Confidential Health  Information (PHI/CHI) under the Standards for Privacy of Individually Identifiable Health Information,  45 C.F.R. §§160.103, 164.501, etc. seq., as amended from time to time, includes examination  findings, test results, and treatment provided for purposes of medical evaluation of the participant,  follow-up and communication with the participant’s parents or guardian when applicable, and/or determination of  the participant’s ability to continue in the festival activities. 

I have carefully considered the risk involved and hereby give my informed consent on behalf of myself or my child to participate in all age-appropriate activities offered at the festival. I further authorize the sharing  of the information on this form with any festival volunteers or professionals who need to know of  medical conditions that may require special consideration in conducting scheduled activities. 

With appreciation of the dangers and risks associated with festival activities and camping, on my own behalf and/or on behalf of my child, I hereby fully and completely release and waive  any and all claims for personal injury, death, or loss that may arise against the UNTAMED festival, the Northern Colorado Covenant of the River (NCCR) council, board of directors, the activity coordinators, and all employees, volunteers,  related parties, or other organizations associated with any festival program or activity. 

I also hereby assign and grant to the UNTAMED festival, as well as their  

authorized representatives, the right and permission to use and publish the photographs/film/ videotapes/electronic representations and/or sound recordings made of me or my child at all festival activities, and I hereby release the UNTAMED festival, the Northern Colorado Covenant of the River council, board of directors, the activity  coordinators, and all employees, volunteers, related parties, or other organizations associated  with the activity from any and all liability from such use and publication. I further authorize the  reproduction, sale, copyright, exhibit, broadcast, electronic storage, and/or distribution of said  photographs/film/videotapes/electronic representations and/or sound recordings without limitation  at the discretion of the UNTAMED festival, and I specifically waive any right to any compensation I may have for  any of the foregoing. 

Covenant of the River is concerned about the safety of its attendees and volunteers while participating with Untamed Fest (whether on or off premises), the safety of its constituents (youth, community, staff, etc.) and the protection of its assets and reputation. To reduce these risks, prospective and, as applicable, current attendees, volunteers and staff will undergo a criminal background check that complies with the Fair Credit Reporting Act (FCRA). Volunteers are treated as applicants under the FCRA. The Registration Manager will review the report and determine if any negative information has a direct connection with an applicant’s ability to participate with competence and integrity, or that might impact safety. Matters that might raise a concern include but are not limited to the following: Sex offenders. I authorize Untamed Fest hosted by Covenant of the River to conduct a complete criminal history check as a basis of my participation as an attendee or volunteer with the organization. I understand that failure to comply with, or negative results will forfeit my ticket and there will be no refund on ticket sales. 

NOTE for MINORS: Due to the nature of festivals, camping, and activities the UNTAMED festival and Northern Colorado Covenant of the River cannot continually monitor compliance of festival  participants for any limitations imposed upon them by parents or medical  providers. However, so that leaders can be as familiar as possible with any limitations, list any restrictions imposed on a child participant in connection with  programs or activities below.

By Purchasing your ticket you agree to the terms and conditions and will be asked to provide a signed copy of both your medical release from and your liability consent form upon arrival at the festival.
